Create an Issue from the web application when a reported equipment condition or inspection finding requires investigation and ownership.
For example, during a round, you notice that Pump A is malfunctioning and
needs attention.
To create an issue:

Click the Observations section on the left pane.

Click the Issues tab.

Click Create Issue.

In the Create Issue screen, provide the following information:
- Enter the issue title in the Name field.
- Enter the issue description in the Description
field.
- Select the Category as
Observation,
Maintenance, Incident,
Near Miss, or Hazard
from the list.
- Select the Priority and select the
Event Execution as
Shutdown, Turnaround,
or General from the list.
Note: If you select the
Category as
Maintenance, the issue is converted to a
notification automatically.
- Add images in the Photos field.
Photos taken or
uploaded are automatically stamped with the time and location,
helping verify when and where each image was captured.
- Select the Plant, Unit,
Functional Location, and
Asset values from the respective
dropdowns.
- In the Assigned To list,
- Choose User and select the user from the
drop-down.
- or
- Choose User Group and select the user
group from the drop-down.
- Select Start Date and
Time.
- Select Due Date and
Time.
- Select the Status as Open,
In-Progress, Resolved,
or Unresolved.

Enable Convert to ERP Notification toggle if you want to convert the
issue to notification.

Click Create.
The issue is created successfully. The supervisor and maintenance team receive notifications about the new issue.
The Issue is created and is available for assignment and tracking.
