Create an Issue
Create an Issue from the web application when a reported equipment condition or inspection finding requires investigation and ownership.
For example, during a round, you notice that Pump A is malfunctioning and needs attention.
For the wider decision between an Issue, an Action and an ERP notification, see Follow Up on Findings.
- Click the Observations section on the left pane.
- Click the Issues tab.
- Click Create Issue.
-
Fill in the Create Issue screen.
Name, Description, and Photos describe the condition. Category and Priority decide what happens to the Issue next. Settle those two before you fill the form.
Category changes where the Issue goes. Choosing Maintenance converts the Issue to an ERP notification automatically. It leaves mRounds and is worked in the maintenance system.
Observation, Incident, Near Miss and Hazard keep the Issue in mRounds, tracked and closed by your team.
Priority — check the risk matrix first. Determine Priority from the risk matrix before you select a value.
For Shutdown and Turnaround priorities, selecting the consequence makes Impact, Likelihood and Spare availability available, and those are carried into SAP.

Select the Plant, Unit,
Functional Location, and Asset.
Search and drill through the location hierarchy to find the right asset or functional location rather than scrolling a flat list.
If the condition has already been reported: mRounds checks for existing Issues on the same asset or location before you complete the form. When open Issues exist for the same asset or location, a message shows them before you continue. Creating from a round task, the message appears when the Issue screen opens; creating independently, it appears once you have selected the asset and location. The check covers ERP notifications as well as mRounds Issues, and resolved Issues can be filtered out.
